Please use the links below to download and print the necessary registration forms for grades 3-5 for the 2011-2012 school year. You may bring completed forms when you register your child on July 28, 2011 from 8:00 - 4:00. You will also have an opportunity to meet your child's teacher during registration. Teachers will also be available to help process forms on Registration Day.
